Forms

When you enter our office, we will ask you to fill out a few forms.  These provide us with general information about yourself and your condition.  You may now print out this patient information form,
fill it out and bring it with you to your first appointment.

Consultation

Once the forms have been completed, the doctor will consult with you to discuss your health-related problems, concerns, as well as potential treatment options.

History and Examination

The doctor will ask you various questions related to your condition to determine your actual problem.  To determine which tissues are affected and to what extent, a number of specialized tests will be performed.

X-Rays

To rule out more serious conditions or assist the doctor is developing the most effective treatment plan for you, x-rays may be required.

Home Instructions

Before you leave, you may be given instructions on certain activities or procedures to be conducted at home.  Home exercises and/or stretches, avoidance of certain activities or positions, or application of ice or heat may be included.

Follow-Up Visit

A follow-up visit will be needed for a complete review of findings and discussion of treatment options.  This is usually scheduled within one to two days of your first visit.
